Ensuring Every Baby is a Healthy Baby
Children’s Services Council of St. Lucie County is issuing this Targeted Priority Funding Opportunity to identify qualified Providers who are capable of implementing a strategy to reduce infant mortality and increase access to prenatal care.
Funding available for this priority is up to $200,000 for the period of October 1, 2019, through September 30, 2020. No strategy will be funded for 100% of expenses. Applying agencies will need to demonstrate buy-in to the strategy. The selected strategy will be funded on a monthly reimbursement basis and will be required to submit quarterly outcome and demographic reports as detailed in the annual contract.
Scope of Work
The Children's Services Council of St. Lucie County, along with many of its community partners, has developed a system of care for newborns and their parents to ensure every baby is a healthy baby. While the system of care has been largely successful in positively impacting families in our community, we realize that additional strategies to the system of care need to be implemented to reach ALL families in our community. To be a partner in the system of care requires collaboration beyond just referrals so that the system is enhanced and improved. Therefore, the Children's Services Council is looking for strategies that address the needs identified in the St. Lucie County Fetal and Infant Mortality Review that reduce infant mortality by increasing access to prenatal care and enhancing infant/family services during the first year of life. The selected strategy will require at least two agencies work collaboratively in their proposed strategy.
